Additional information, updated photos, medical clearance, or referral to a healthcare provider may be required before services can be scheduled.
Compassionate Soles provides non-invasive, preventive foot care services and does not diagnose, prescribe, or treat medical conditions.

This form helps us understand your health history, foot concerns, mobility needs, medications, and service goals. The information you provide allows us to determine the safest and most appropriate care plan for your needs.

✅ Step 2: Complete the Appointment Policies & Client Acknowledgment Form
This form reviews important policies regarding appointments, cancellations, no-shows, mobile visit requirements, payments, and service expectations. Your acknowledgment helps ensure a smooth experience for both you and our team.

✅ Step 3: Submit Clear Foot Photos Before Your Visit
Photographs help us review your foot and nail concerns prior to your appointment and determine whether additional information or referrals may be needed.
Please upload:
✔ Top of the left foot
✔ Top of the right foot
✔ Clear photo of all toenails
✔ Bottom (sole) of each foot if you have concerns involving the soles, calluses, cracks, skin conditions, or pain
Photo Tips:
Use good lighting
Remove shoes and socks
Ensure photos are clear and in focus
Submit current photos taken within the last 7 days
Do not apply lotion or oil before taking photos
